Contest Qualification Steps

1. Complete the Entry Form: Begin by filling out the provided entry form above. Once you’ve submitted it, our guest team will contact you to set up your detailed 90-minute LASIK evaluation. To ensure accurate assessment and custom measurements, please refrain from wearing contact lenses for 2 days before the evaluation.

2. Experience the Monson Vision Ocular Analysis (MVOA): During your evaluation, you'll undergo the MVOA, Monson Vision’s most comprehensive eye examination. Additionally, you’ll have the opportunity to meet personally with Dr. Bryan Monson, MD. He will work with you to develop the best custom treatment plan for your unique eyes.

3. Book Your LASIK Procedure: Make sure to book your LASIK surgery with Monson Vision by February 5th, 2024, to qualify for our grand prize drawing.

Contest Rules

• To be eligible for the contest and win, you will need to submit the form during the New Year, Best You! event timeframe, January 5 - February 5, 2024.
• You must schedule your LASIK procedure at Monson Vision by February 5th, 2024.
• Must be 18+ years of age to enter.
• To qualify, vision deficit must not be attributed to trauma, disease, or presbyopia and should be correctable via laser enhancement, as assessed by one of our surgeons.
• Participants must be medically and surgically safe Laser Vision Correction candidates, as determined by our surgeon.
